Default Submission of names for course / overseas trip

To ease CSO's work, here is a default way to submit your names (FOR CADET OFFICERS ONLY).

Rank and Name:
NRIC:
Contact number (home and mobile):
Address:
Next Of Kin (stat if it is your mum or dad):
Next Of Kin Contact (a working line):
Intake:
A photocopy of the back page of your passport which states the expiry date and your picture (unless for oversea trips).
